Please arrive 15 minutes before your first appointment so that we can collect necessary information from you.

Bring with you to every appointment:

• A photo ID
• Your insurance card
• Your Method of copayment or payment in full if you are uninsured
• A list of medications, including over-the-counter medicines and supplements, you are currently taking
