Request Records Through Healow Patient Portal:

The fastest way to receive records is digitally through the patient portal. Existing patients who have signed up for the Healow Patient Portal may log in to view medical records. New patients may create a Healow Patient Portal. If you need assistance, please contact Chicago ENT at 773-296-5500.

Patients and patient proxies can request records through their Healow account. Requests can only be made for yourself, as the records are delivered through the Healow Patient Portal. Images cannot be requested in this manner. Any other requests must be made by completing an authorization form.

Follow these steps to request additional records uploaded to your portal:

From your Healow portal select Messages –> Inbox –> Compose. Add the subject of “Medical Records” and include what records you are requesting (ex: All records, a specific test or lab result, or a range of dates of service). 

All Other Record Requests

A release form is required to allow Chicago ENT/Chicago Sleep Center to release your health information. 

1. To have your records mailed to yourself or another provider, please fill out this form.
   1. Form does not exist yet
   2. *Make address a required field, remove fax
   3. To mail to self, have a checkable option to “send to self” after patient enters their address
   4. To mail to another provider, include address spot
2. Fill out the whole form including the kind of records requested and dates of your visits.
3. Click “Submit” to send your form to us.

Alternatively, you may fill out the PDF release form and return it via email, fax, or mail with any required documents to:
